Score
A score question takes an ordered array of levels in criteria. Each level can be a string or a structured value containing phrases to locate. The response reports the selected level and its zero-based index; it does not interpolate a numeric score between levels.

score is the selected declared level, not a continuous or expected value. When more than one level is supported, the result is unresolved; when none is supported, it is refused. Both outcomes set may_act to false.
