Questions and criteria

POST /v1/systemone accepts a state plus a questions object. Each question has a type and declared criteria:

`choice`	An object keyed by option name; values describe the declared criteria.	`choice`
`score`	An ordered array of levels.	`score` and zero-based `index`
`noul`	An object with `true` criteria and optional `false` criteria.	`noul`, set to `yes` or `no`

Question criteria are declarations supplied by the caller. Strings are split on commas, semicolons, or, and and; arrays and objects are read recursively. The not_for object field is excluded from positive criteria. The actual decision path can include a configured engine stage, which is named in the response where available.
