Handle decision outcomes
Branch on the documented outcome returned by the engine. Do not route based on a confidence threshold: confidence fields, when present, are compatibility metadata and are not a calibrated gate. Tier 3 is called only when the request explicitly includes supervisor or finalize.

An application may handle a decided answer by checking its own policy and executing the returned call. For unresolved, ask the user to choose or provide more information. For refused, follow the refusal reason or stop. The result is not itself an authorization decision.
