Read from configured documents

The /v1/read endpoint can return an answer with a located source span when a reader is configured for the serving stack. On the current clean stack, conformance capture returns outcome: "unavailable" because no reader is configured. Do not present this endpoint as an active retrieval service until the serving stack includes and verifies a reader.

Applications must handle unavailable without claiming that a document was searched or an answer was verified. When a reader is configured, verify the deployed stack and captured response before relying on its behavior. See decision outcomes for the distinction between a decision and an application action.
